How to write a thank you and goodbye letter?

I was given a privilege to play in an orchestra at a high school of which I was not enrolled. The conductors were great, they helped me so much with my playing and advanced my level very quickly. Now after 1 year of playing with them I am leaving to another state (the original plan was to play with them for another year.) I want to write the conductor a thank you letter (and give him a gift basket.) What all should I include in my letter?
--------------------
Dear sir, It was my great luck and pleasure to have this experience of playing in our high school orchestra. And I am quite surprised at my quick advancement in my levels which is all contributed by the great conductors who have helped me so much..... I love our orchestra so much that had initially planned to play with them for another year but due to some personal reasons I am leaving for another state and I would definitely miss our orchestra the most. I thus sadly wish all our team a goodbye and warm wishes and would love to share that this brief period of one year of my life has become one of the most pleasurable and memorable year due to the wonderful days i spent in our orchestra.........it was really a life changing experience. Good Bye and Wish you all the best.
